If you have problem seeing the upload window, this could probably be caused by you are signed in to many accounts on the same browser. This caused Google unable to recognize which drive to open because there are more than one account available.
To rectify this, you need to sign out from all Google accounts and then sign in to the account you want to use to submit your file. Then, refresh the submission page and fill the form.
Or you may also switch the account on the Submission Form and fill the form as usual.
